If you already have a 403(b) with us and would like to change your contribution amount, please just email us. If you would prefer to complete the form yourself, please click on the OMNI form below. You can return it to us or to your district's payroll specialist. Most (but not all) local districts use The OMNI Group for payroll deduction administration.
For your convenience, we have also included our Address Change and Beneficiary Change forms below. Forms should be returned to our Fairport office for processing.
